Важно отметить, что ребенок в момент переливания эритроцитарной взвеси находился после полостной операции в тяжелом состоянии, обусловленном интоксикацией, тяжелой анемией, синдромом прооперированного кишечника и сепсисом в послеоперационном периоде. Эти обстоятельства, а также значительный уровень РНК ВИЧ в крови донора более 40 тысяч копий/мл предполагали высокий риск инфицирования реципиента. Отсутствие утвержденного порядка проведения профилактических мероприятий в данной клинической ситуации и небольшой опубликованный опыт обусловили сложности ведения данного случая. Обсуждаемая в статье клиническая тактика ведения пациента, и представленная положительная динамика показателей лабораторного обследования могут быть полезными для ученых и клиницистов.</p></abstract><trans-abstract xml:lang="en"><p>A unique clinical case of successful prevention of HIV infection after transfusion of erythrocyte suspension contaminated with HIV to a 1-year child is described. Importantly, the condition of the child being transfused was severely bad after abdominal operation complicated with intoxication, severe anemia, postoperative bowel syndrome, and sepsis. All this in combination with the high level of HIV RNA in donated blood (more than 40 000 copies per 1 mL) made the risk of HIV transmission very high. The absence of an established procedure for preventive interventions under such conditions and a limited published experience of dealing with such cases made the management of this case still more complicated.
