В рамках ретроспективного анализа информации регистра региональной системы мониторинга туберкулеза изучены данные 33 414 пациентов с ВИЧ-инфекцией, впервые обратившихся в кабинет врача-фтизиатра в 2016–2023 гг. Длительность наблюдения составила от 1 года до 8 лет. В 2016–2023 гг. среди пациентов было выявлено 745 случаев заболевания туберкулезом. Для решения задач исследования были сделаны три выборки. Проведена оценка характеристик пациентов, обращавшихся к фтизиатру в 2016–2023 гг., на основе данных 31 614 пациентов с ВИЧ-инфекцией, у которых в процессе наблюдения не был выявлен туберкулез или подозрение на туберкулез при первичном скрининге, и отсутствовал факт нахождения на учете в противотуберкулезном учреждении как больного активными формами заболевания. Для оценки выявления ЛТИ при первом визите в кабинет проведен анализ данных 25 848 пациентов, у которых при первом обращении, зарегистрированном в 2016–2023 гг., не был выявлен туберкулез, и были известны результаты иммунологического теста с использованием аллергена туберкулезного рекомбинантного. Среди 24 581 пациента из указанных выше лиц, у которых при первом обращении в кабинет ЛТИ не была установлена, проведена оценка выявления ЛТИ при повторных визитах к фтизиатру. Для анализа заболеваемости туберкулезом в зависимости от проведения превентивной терапии (ПТ) в 2017–2023 гг. были рассмотрены данные 30 195 пациентов, которые первый раз обратились к фтизиатру в 2016–2022 гг. и у которых в анамнезе отсутствовали случаи заболевания туберкулезом как до первого посещения, так и в течение первых трех месяцев после первого посещения. Сравнивали годовую заболеваемость туберкулезом у пациентов, не получавших ПТ, с временем наблюдения в течение года через 90 дней после первого обращения к фтизиатру; и получавших ПТ, с временем наблюдения в течение года через 6 мес после начала курса ПТ.</p><p>Результаты и их обсуждение. В 2016–2023 гг. при первом обращении к врачу доля лиц с ЛТИ колебалась в диапазоне от 3,4% (в «ковидный» год пандемии новой коронавирусной инфекции) до 5,2%, при среднем значении показателя 4,6% (95% ДИ 4,3–4,8%). При повторных посещениях к врачу появление ЛТИ отмечали ежегодно в диапазоне 2,2–2,8% лиц. Сравнение годовой заболеваемости туберкулезом двух групп: не получавших и получавших превентивную терапию показало, что превентивная терапия против туберкулеза снижает шанс заболевания туберкулезом почти в  два раза (ОШ=1,9, 95% ДИ 1,2–2,9). Для лиц, не получавших антиретровирусную терапию, шанс заболеть туберкулезом уменьшается в 3,5 раза (ОШ=3,5, 95% ДИ 1,3–9,4). У пациентов с уровнем CD4+ лимфоцитов менее 350 кл/мл вероятность заболевания снижалась в 3,5 раза (ОШ=3,5, 95% ДИ 2,0–6,1). У пациентов с ВИЧ-инфекцией/ЛТИ при отсутствии проведения полного курса превентивной терапии туберкулеза шанс заболеть туберкулезом повышался более чем в 4 раза (ОШ=4,2, 95% ДИ 1,5–11,7).</p></sec><sec><title>Заключение</title><p>Заключение. На большом массиве данных о пациентах с ВИЧ-инфекцией, посетивших фтизиатра, было оценено распространение ЛТИ и показана статистически достоверная эффективность ПТ, особенно у пациентов с низким уровнем CD4-лимфоцитов и ЛТИ.</p></sec></abstract><trans-abstract xml:lang="en"><p>The aim of the study was to study the prevalence of latent tuberculosis infection (LTBI) and the incidence of active forms of tuberculosis (TB) depending on the implementation of preventive therapy for tuberculosis (PT) in patients with HIV infection.</p><sec><title>Materials and methods</title><p>Materials and methods. As part of a retrospective analysis of the registry information of regional TB surveillance system, the data of 33,414 patients with HIV infection who first visited a tTB doctor in 2016–2023 were studied. The observation period ranged from 1 to 8 years. In 2016–2023, 745 cases of tuberculosis were identified among patients. To solve the research aims, three samples were made. An assessment of the characteristics of patients who visited a tuberculosis specialist in 2016–2023 was carried out based on the data of 31,614 patients with HIV infection who were not TB diagnosed or suspected of having tuberculosis during the initial screening and who were not registered with a TB institution as a patient with active forms of the disease. To assess the detection of LTBI during the first visit to the office, an analysis of data from 25,848 patients was performed, in whom tuberculosis was not detected during the first visit registered in 2016–2023, and the immunological test results using the recombinant tuberculosis allergen were known. Among 24,581 patients from the above-mentioned persons, in whom LTBI was not detected during the first visit to the office, an assessment of LTBI detection was performed during repeat visits to a TB specialist. To analyze the incidence of tuberculosis depending on the implementation of preventive therapy (PT) in 2017–2023, data from 30,195 patients who first visited a TB specialist in 2016–2022 and who had no TB history either before the first visit or during the first three months after the first visit were reviewed. The annual incidence of tuberculosis was compared in patients who did not receive PT, with a one-year observation period 90 days after the first visit to a TB doctor; and in those who received PT, with a one-year observation period 6 months after the start of the PT course.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. In 2016–2023, the proportion of people with LTBI at the first visit to a doctor fluctuated between 3.4% (in the «covid» year of the COVID-19 pandemic) to 5.2%, with an average value of 4.6% (95% CI: 4.3–4.8%). During repeat visits to a doctor, LTBI was detected annually in 2.2–2.8% of people. Comparison of the annual incidence of tuberculosis in two groups: those who did not receive and those who received preventive therapy showed that preventive therapy against tuberculosis reduces the chance of developing tuberculosis by almost half (OR=1.9, 95% CI: 1.2–2.9). For people who did not receive antiretroviral therapy, the chance of developing tuberculosis decreases by 3.5 times (OR=3.5, 95% CI: 1.3–9.4). In patients with CD4+ lymphocyte levels less than 350 cells/ml, the probability of developing the disease decreased by 3.5 times (OR=3.5, 95% CI: 2.0–6.1). In patients with HIV/LTBI, in the absence of a full course of preventive therapy for tuberculosis, the chance of developing tuberculosis increased more than 4 times (OR=4.2, 95% CI: 1.5–11.7).</p></sec><sec><title>Conclusion</title><p>Conclusion: Using a large data set of HIV-infected patients visiting a TB specialist, the prevalence of LTBI was assessed and statistically significant effectiveness of PT was demonstrated, especially in patients with low CD4+ levels and LTBI.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>ВИЧ-инфекция</kwd><kwd>туберкулез</kwd><kwd>профилактика</kwd><kwd>латентная туберкулезная инфекция</kwd><kwd>превентивная терапия</kwd></kwd-group><kwd-group xml:lang="en"><kwd>HIV infection</kwd><kwd>tuberculosis</kwd><kwd>prevention</kwd><kwd>latent tuberculosis infection</kwd><kwd>preventive therapy</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">WHO.
