Ее дисфункция на ранних стадиях заболевания проявляется в виде гиперкортизолемии, которую связывают с повышением уровня цитокинов, индуцирующих продукцию глюкокортикоидов. По мере прогрессирования ВИЧ-инфекции увеличивается вероятность развития надпочечниковой недостаточности в результате оппортунистических инфекций (чаще цитомегаловирусной) или неопластической инфильтрации надпочечников и/или гипофиза. Лекарственные препараты, использующиеся при терапии ВИЧ-инфекции, также могут способствовать развитию адреналовой недостаточности. Для дифференциальной диагностики состояний, напоминающих надпочечниковую недостаточность, и явной надпочечниковой недостаточности и для оценки резервной способности надпочечников необходимо выполнять диагностические тесты, поскольку необоснованная терапия глюкокортикоидами является опасной.</p></abstract><trans-abstract xml:lang="en"><p>HIV infection is often accompanied by disorders in the hypothalamus-pituitary-adrenal system. Its dysfunctions at the early stages of the disease are manifested as increased blood cortisol, which is believed to be caused by increased cytokines able to enhance glucocorticoids production. With the further progression of HIV infection, the risk of adrenal failure increases because of opportunistic infections (most commonly cytomegaloviral) or neoplastic infiltration of adrenals and/or pituitary. Drugs used to treat HIV can also promote adrenal failure.
