Проанализированы частота и степень выраженности нежелательных явлений при использовании атазанавира или абакавира в схемах АРВТ в период с 2004 по 2013 г. в зависимости от выявленных генетических особенностей. Частота повышения концентрации непрямого билирубина 3-4-й степени при применении ATV составила 36,2%, выраженной желтухи - 10,6%. Наибольшему риску развития выраженной гипербилирубинемии были подвержены пациенты с гомозиготным генотипом UGT1A1*28. Аллель UGT1A1*28 выявили у 57,4% больных, а гомозиготный аллель - у 14,9%. HLA B*5701 был обнаружен у 13 больных (3,7%). При условии проведения тестирования частота отмены ABC была крайне низкой - 4,7%. Специфичность теста составила 100%, а чувствительность и информативность - 99,2%.</p></abstract><trans-abstract xml:lang="en"><p>The presence of the alleles UGT1A1*28 and HLA B*5701 was checked up in 47 and 366 adult HIV patients, respectively. The rate and severity of the adverse effects of using Atazanavir or Abacavir in ART during 2003-2013 were compared with the genetic findings. The prevalences of grade 3 or 4 increases in unconjugated bilirubin and of overt jaundice upon ART were 36,2% and 10.6%, respectively. The highest risk of profound hyperbilirubinemia was featured by patients homozygotic for UGT1A1*28. This allele was found in 57,4% of patients, including 14,9% homozygotes. HLA B*5701 was found in 13 patients (3,7%). Taking account of testing results decreased the rate of ART discontinuation to 4,7%.
